In case of submitting in person: 1. Legalization Application Form [Download] 2. Letter of Consent and Power of Attorney forms [Download] with one set of copy 3. A copy of the minor’s Thai birth certificate or a copy of the minor’s Thai house registration (In case of adopted child, please provide an evidence of adoption) with one set of copy 4. Original valid Thai ID card or passport (if the father or mother is a foreigner, please submit a valid passport) along with one set of copy |
In case of submitting by postal mail, please prepare the above mentioned documents and additional documents as follows: 1. Notary Public to Notarize a Signature form [Download] – You must sign with the Notary Public officer to certify your signature. 2. If you cannot send the original Thai ID or passport, please have a copy of this document and notarized by a Notary Public 3. Return envelope with a tracking number or USPS Priority Mail Flat Rate (Meter Stamp is not accepted) and clearly address the envelope to yourself 4. Send all the required documents to Royal Thai Embassy (Attn: Legalization) 2300 Kalorama Road NW, Washington D.C. 20008 ***Remark: Royal Thai Embassy is not responsible for any documents lost in the mail, under any circumstances.*** |
