Required Documents for Applying for an Emergency Passport (EP)

 

1. A letter explaining the reasons for emergency travel Download

2. Evidence demonstrating the necessity for emergency travel, such as a medical certificate or any other official documents.

3. A form for requesting an Emergency Travel Document certified by a Notary Public and its copy Download

*If the person is under 20 years of age, the father and mother must sign on the Notary Public for Signature form*

4. A Thai passport (except of loss or theft)

5. A Thai national ID card or house registration (can be replaced with a copy if you do not wish to submit the actual ID card to the officer)

6. A copy of the airfare ticket (can be sent via email at a later time)

7. A police report from a local police station and its copy (only in case of loss or thef)

9. 2"x2" of a color photograph taken within the last 6 months, with your name written on the back of the photograph. The photograph should be a front view, stand against a white background, dressed appropriately with sleeves. No hats, glasses, and smiling with teeth showing, and please glue the photograph onto the emergency travel document application.

10. Priority Mail Express Flat Rate Envelope with postage stamps 30.45 USD addressed to yourself for the officer to return the emergency travel document to you.

11. The fee of 8 USD must be paid only by Money Order or Cashier's Check, payable to the Royal Thai Embassy (cash, personal checks, and credit cards are not accepted).

12. In the case of an applicant under 20 years old, the following additional documents are required (copies may be used if you do not wish to submit the original documents):

  • A birth certificate
  • A marriage certificate/divorce certificate of the parents
  • Child custody documents (in case of divorce or if not married)
  • A National ID card or passport of the parents
